Circumcision is a procedure that removes the foreskin that hangs over the head of the penis. For many years, circumcision of newborns was routine in the United States. Because of concerns about the pain that might be experienced by an infant undergoing this minor procedure, the practice of circumcision decreased for many years. It is on the rise again in many areas.
WHY DO MEN AND ADOLESCENTS DECIDE TO HAVE A CIRCUMCISION?
There are many reasons. What follows is a list of the more common ones:
- Persistent infections of the foreskin
- Reducing the risk of STDs like HIV, HPV (venereal warts) and Herpes
- Pain, bleeding or cracking of the foreskin with erections
- Difficulty with foreskin retraction
- Cosmetic reasons
- Premature ejaculation, urinary tract infections (UTIs) or pain and bleeding of the foreskin during erections.
- Protection against penile cancer
- Decreasing the incidence of cervical cancer in female partners
ARE THERE ANY OTHER BENEFITS TO A CIRCUMCISION?
In many men, low grade, chronic infections under the foreskin cause periodic pain, burning or even a discharge. Hygiene is easier to maintain after a circumcision. Men who get urinary tract infections (UTIs) may see a significant reduction or elimination of them after a circumcision.

HOW IS A CIRCUMCISION PERFORMED?
At Pacific Coast Urology’s Medical Center of Excellence for Circumcision, Dr. Pugach uses a freehand method because he feels it achieves a cosmetic result that can be significantly superior to the old-fashioned clamp method. It allows preservation of frenular tissue to enhance sensation and can prevent irregular scarring. It can also preserve more natural penile tissue.
A circumcision is minor surgery that is performed either with a general anesthetic (preferred) or with a local anesthetic. The pros and cons to these will be discussed in detail with you when you come in for your consultation.
The procedure takes approximately 40 minutes to perform and you will return home shortly after your procedure.
WHAT CAN I EXPECT AFTER MY CIRCUMCISION?
Patients typically experience some swelling and mild pain or discomfort for the first 24 to 48 hours. Most normal activities can be resumed in a few days. We recommend that you wear loose pants for a week or so. The stitches that are used will come out by themselves in 7 – 14 days. Dr. Pugach advises all patients to avoid sexual activity for 4 weeks after the procedure.

Circumcision may be covered by your health insurance plan. Pacific Coast Urology Medical Center takes most PPOs and a limited number of medical groups for patients with HMO coverage. We will check your insurance benefits without charge for you. However, we cannot guarantee that the information your insurer provides to us is completely accurate so we also recommend that you personally check your benefits with your insurer.

If the procedure is not covered or you do not have health insurance, Pacific Coast Urology Medical Center offers an affordable cash package for your circumcision. We accept cash, Visa, MasterCard, Discover, or a cashier’s check for payment. While we do not offer financing, you may send pre-arranged payments in advance. Once Pacific Coast Urology Medical Center has received full payment we can schedule your circumcision.
All of the surgical fees are inclusive – there are no hidden or extra charges and they include all routine post-operative visits.
