My patients and readers of my articles and blogs are very familiar with my strong stance on the absolute need for annual prostate cancer PSA screenings to diagnose and treat prostate cancer. I have never been a proponent of the ill-advised recommendation of the United States Preventive Services Task Force (USPSTF) to stop routine PSA screenings

A little history is in order. In 2008, USPSTF ignited controversy in the medical community when they advised against most routine screening for prostate cancer with a PSA blood test. In 2012, the USPSTF went on to issue guidelines against PSA screenings. This recommendation set off a firestorm of debate in the urological community and I joined my colleagues in condemning this recommendation. Most urologists are advocates of annual prostate testing with a PSA and a rectal examination.

In the ensuing years, there have been a number of studies illustrating that the increase in prostate cancer deaths can be linked to a slowdown in routine PSA screenings. Now, definitive evidence is in! A January 2021 study, “Association of Reductions in PSA Screening Across States with Increased Metastatic Prostate Cancer in the United States,” establishes the fact that the rise in metastatic prostate cancer is linked to PSA screening reduction. At the 2021 Genitourinary Cancers Symposium, investigators reported the following finding, “In states with larger decreases in PSA screening, there was a larger increase in the incidence of metastatic prostate cancer at diagnosis.”1

I have one of the largest prostate cancer practices in the world. My goal is to diagnose prostate cancers at an early stage so they can be treated with one of the minimally invasive options that I have helped to develop. A report issued on April 5, 2016 at the American Society for Nutrition annual meeting in San Diego, is once again bringing attention to whether or not sugary drinks and carbs are linked to prostate and breast cancers.

Is There a Correlation with Soda Causing Prostate Cancer?

The study’s lead researcher Nour Makarem, a Ph.D. candidate in nutrition at New York University, explained the link her team found between sugar-sweetened drinks (both soda and fruit juice) and prostate cancer risk.  The research compared with men who never drank sugary beverages to those who had them a few times a week. The finding showed a 3 times greater risk of developing prostate cancer as compared to men who stay away from these drinks.  The study also showed that men who at a higher intake of processed foods (pizzas burgers, processed deli meats) doubled their prostate cancer risk.

How Do You Know How Much Sugar is Too Much Sugar?

Doctors, nutritionists and researchers alike have varying dietary recommendations*.  According to Marji McCullough, strategic director of nutritional epidemiology for the American Cancer Society, “it is difficult to weed out the effects of particular diet habits on cancer risk.  “Few dietary factors apart from alcohol and/or obesity have been consistently related to postmenopausal breast cancer and prostate cancer,” McCullough said.

What are Some of Prostate Cancer Risk Factors?

While the causes of developing prostate cancer are unknown, some risk factors for the disease have been identified:

  • Advancing age
  • Genetics
  • Heredity
  • Hormonal influences
  • Dietary
  • Environmental factors

However, it is important to note that I and most experts agree – limiting intake sugary drinks and processed carbohydrates, and eating more fruits, vegetables, legumes, fiber-rich whole grains and “good” unsaturated fats is a good health decision.   Dr. Robert Pugach concurs with this recommendation.

American Society for Nutrition Study on Sugar Drink / Carbs Link to Prostate Cancer

  • Based on nearly 3,200 U.S. adults whose diet habits and cancer rates were tracked for 20+ years.
  • During that time, 565 people were diagnosed with cancer.
  • The study was limited to one demographic group that is not reflective of the population of the United States. Most study participants were Caucasian.
  • *The authors of this study abstract acknowledge their findings do not show that beverages cause any disease,” the group said in a statement.

On March 12, 2016, Dr. Pugach and Robert Colvin of HIFU Prostate Services* welcomed physicians to the Los Alamitos Surgical Center to learn more about High Intensity Focused Ultrasound (HIFU).  The purpose of this educational open house was to educate physicians on how HIFU works and the advantages of the HIFU procedure for prostate cancer.  HIFU can cure cancer without the two common, life-altering side effects that radical surgery (robotic or traditional) and radiation can cause.

After treating hundreds of prostate cancer patients with High Intensity Focused Ultrasound (HIFU) internationally, Dr. Robert Pugach will be treating his first Sonablate™ HIFU patients in the United States, on February 16, 2016 at the Los Alamitos Surgery Center in Los Alamitos, CA.

For 10+ years, Dr. Pugach has regularly travelled out-of-the country to treat his prostate cancer patients with a remarkable advanced technology, HIFU (High Intensity Focused Ultrasound).  HIFU has been used in 40+ other countries for more than 15 years and was approved by the FDA in October, 2015 for use in the U.S.

“HIFU fits perfectly within the focus of my minimally invasive urologic practice as small prostate cancers can often be treated in less than an hour and patients can return to normal activities within a few days with a dramatic reduction in quality of life issues like erectile dysfunction (ED) or incontinence,” says Dr. Pugach.

George, a 67 year old Bellflower resident,  will be one of the first patients treated with Sonablate™ HIFU at the Los Alamitos Surgical Center. “I decided to have HIFU  because I understand it’s the best procedure and Dr. Pugach already ‘fixed’ my friend, John, a few months ago, says George.  “He’s doing great and I look forward to the same experience!”

While HIFU isn’t currently covered by Medicare or most health insurance plans, Dr. Pugach explains, “men have elected to pay for this procedure so that they can retain their quality of life.” “Historically, the conventional treatment for prostate cancer is the surgical removal of the prostate and, while this surgery is now done robotically, it still has the potential to cause significant, life altering side effects such as permanent urine leakage and erectile dysfunction (impotence),” continues Dr. Pugach.

HIFU technology has been clinically tested for use to treat a wide variety of diagnoses, including localized whole gland prostate cancer, benign prostatic hyperplasia (BPH) and recurrent prostate cancer.
