We participate in the Medicare program and will submit your claim directly to Medicare. You are responsible for your deductible and co-insurance.

Please bring your Medicare or health insurance identification card to your visit at Pacific Coast Urology Medical Center. Your insurance may require prior authorization before being seen by Dr. Robert Pugach. If required by your health insurance, please ask your primary care doctor or insurance company to fax your authorization to 562.594.9010 to ensure you aren't billed for services covered by your insurance. You may be called for insurance or authorization information before your appointment.

Your insurance may require an office co-payment, if so, you will be asked to make your co-payment in full at the time of your visit. If your visit is not covered by a health plan or insurance, you will be asked to pay for your visit on arrival. For your convenience, we accept cash, checks and credit cards. We offer cash packages for procedures such as vasectomy, vasectomy reversals, and circumcision. Please ask us about these special cash packages.

In an effort to simplify the handling of your insurance claim, we will provide you with a monthly statement and information that you may need for insurance purposes. We acknowledge your need for a clear understanding of the financial arrangements for your medical visits and surgical care. Therefore, a discussion of fees for planned diagnostic or surgical procedures is welcomed. Please check our financial counselor to verify our participation in your plan or if you have any other insurance or billing questions.
